Problems with O, V6.1.2

Dear O-users,
              currently I'm using O, Version 6.1.2 on an SGI with an R10000 
processor, 64 Mb main memory, and solid impact graphics. Quite a lot of strange 
things seem to happen: 

    When I read in a new version of the user menu (by "read mymenu.o"), the 
new menu is properly displayed during the current session, but not saved, 
i.e. at the next session again the old user menu is displayed and I've to read
the file again. Do I have to kill a database before reading in a new user menu?  

    Although I frequently click on the "SAVE" option and exit the program with 
STOP, it seems as if changes to the coordinates are not always saved. No error 
message is printed either. How can I make sure that all manipulations made to 
the coordinates are always saved?

   Being in stereo mode the program crashed (not for the first time) with 
the message "segmentation fault" and a core being dumped. Worse than that it 
created a new binary O-file at the moment of crash, with a slightly different 
name ("hppd61.o") than the one I started it with ("hppd.o"). I could restart 
O with this new file but all changes to the model were lost (even those made 
before the last "SAVE"). On the other hand it remembered the new user menue 
this time. What could have happened here and how could I avoid it?

I know all this sounds a bit bizarre, but any useful hint will be appreciated.
